Jacksonville Dermatology & Cosmetic Surgery (“JDC,” “we,” “our,” or “us”) respects your privacy. This Privacy Policy explains what information we collect through our website at cosmeticdermatologyjax.com, how we use it, and the choices you have.

This policy applies to our website only. Information you share with us as part of your in-person care — including your medical records, treatment history, and anything you discuss with our providers — is protected separately under federal and state health privacy laws. Information we collect

Information you provide. When you fill out a form on our website — such as a contact form, appointment request, or patient portal inquiry — we collect the information you enter. This typically includes your name, phone number, email address, and any message you choose to send us.

Information collected automatically. When you visit our website, certain information is collected automatically by our analytics and advertising tools:

We do not collect form field contents, chat contents, or the text of anything you type into our website through these automatic tools. Our analytics are configured to redact email addresses and common personal identifiers from any web address before the data is sent to Google.

Third parties that receive data from our website

  • Google Analytics 4 — website usage analytics. Configured for minimum-necessary data collection: email and personal identifier redaction is enabled, cross-device user tracking (“Google Signals”) is disabled, and data is retained for two months.
  • Google Ads — measures whether visitors who clicked our online ads went on to call us or submit a form. We do not record audio of patient phone calls through this system, and we do not share call data with third-party call-tracking vendors.
  • Google Maps — embedded map showing our office location.
  • Our website hosting and content delivery providers — these process standard web traffic (for example, loading pages and images) and keep basic access logs for security purposes.
  • Anti-spam services — we use automated tools to block spam submissions to our contact forms.

We may also share information when required by law, in connection with a legal proceeding, or to protect the safety of our patients, staff, or practice.

Marketing communications

If you’ve provided your contact information and consented to marketing, we may send you information about our services, events, promotions, or educational content by email, phone, or SMS.

You can opt out of marketing communications at any time:

  • Email: click the “unsubscribe” link at the bottom of any marketing email.
  • SMS: reply STOP to any marketing text message. Message and data rates may apply from your carrier.
  • Phone: ask to be removed from our marketing list when we contact you, or call us at (904) 420-7372.

Opting out of marketing does not affect appointment confirmations, care-related messages, or other communications related to your treatment — those are separate.

Do not submit protected health information through our website

Our contact forms and email are not secure channels for protected health information (PHI). Please do not send diagnoses, test results, images of medical conditions, medication lists, insurance card images, or other sensitive medical information through the website or by unencrypted email. If you need to share this kind of information, call our office or use the patient portal, which is linked from the top of our site.

If you do submit health information through our website, we will handle it with care and include it in your patient record once we are able to verify your identity — but the transmission itself is not protected the way the patient portal and in-office records are.

Your choices

Opt out of analytics. You can block our analytics tools by using a browser setting (“Do Not Track” or similar), a browser extension, or Google’s Analytics Opt-Out Browser Add-on.

Request access, correction, or deletion. If you are a patient of the practice, requests related to your medical record are handled under our Notice of Privacy Practices. For information we collected only through the website (for example, a contact form submission that did not become a patient record), you may contact us at the information below to request a copy, correction, or deletion. We will respond within a reasonable time.

California, Florida, and other state law rights. Residents of some states have additional rights under state privacy law, including the right to know what personal information is collected about them, the right to delete it, the right to correct it, and the right to opt out of the sale or sharing of personal information for targeted advertising. We do not sell personal information. To exercise any state-law right, contact us using the information below.

Children

Our website is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the website. Parents and guardians arranging care for a minor should contact the office directly rather than submitting information through our forms.

Data retention

We retain information submitted through our website only as long as needed for the purposes described in this policy. Website analytics data is retained for two months. Form submissions that become part of a patient record are retained according to our Notice of Privacy Practices and applicable medical record retention laws.

Security

We use reasonable administrative, technical, and physical safeguards to protect the information we collect through our website. No website or electronic transmission is completely secure, however, and we cannot guarantee the security of information transmitted over the internet. Please use the patient portal for any sensitive medical communications.

Changes to this policy

We may update this Privacy Policy from time to time. When we do, we’ll update the effective date at the top of the page. Significant changes will be highlighted for a reasonable period of time. Your continued use of the website after changes are posted constitutes acceptance of the updated policy.
